Domperidone
Domperidone blocks action of dopamine. It has strong affinity for D2 & D3 dopamine receptors. Dopamine receptors are found in the chemoreceptor trigger zone, located just outside Blood Brain Barrier, which among others regulates nausea and vomiting.
Apart from it Domperidone also –
- Increases oesophaglal peristalsis.
- Increases tone of lower oesophageal sphincter.
- Increase gastric motility & peristalsis.
- Speeds gastric emptying.
- Reduces small bowel transit time.
GERD
GERD or Gastro esophageal Reflux Disease occurs when the lower esophageal sphincter (LES) opens spontaneously, for varying periods of time, and does not close properly and stomach content rise up into the esophagus.
When refluxed stomach acid touches the lining of the esophagus, it may cause a buring sensation in the chest or throat called heart burn or acid ingetion. Persistent reflex that occurs twice or more in a week is called GERD
Rational of combination
Conditions like GERD, reflex oesophagilis & NUD (Non ulcerative Dyspepsia) has overlapping symptoms and common anatomical etiology (underlying cause). Hence to manage the symptoms and to eradicate underlying causes both domperidone & pantaprazol is required.
- Tone of LES decrease – Domperidone.
- Gastric Motility decrease – Domperidone.
- Acid in stomach increase – Pantoprazole.
Doses & Administration –
GERD – One cap daily empty stomach for 4 to 6 weeks.
Reflex Oesophagitis – One cap daily empty stomach for 6 to 3 weeks.
NUD – One cap daily empty stomach for 4 weeks.
Presentation
10 strips of 10 caps in a box. Domperidone and Pantaprazole , both, are in granules form inside the caps unlike competitors who have kept Domperidone 30mg sustain release tabs.
