Mode of Action – 

Amoxicillin – It is a semi synthetic, pencillin derivative  antibiotic, effective against a wide spectrum of sensitive gam +ve, gam – ve and anaerobic bacterial pathogens including beta – lactamase producing strains. It has high affinity for penicillin binding proteins with varying site of activity. It acts by inhibition of bacterial cell wall synthesis. The elimination half – life is about 3 hrs. With little variation over the usual therapeutic dosage range.

Clauvulanic acid – It is a naturally derived betalactamase inhibitor by streptomyces clavuligerus. It is an irrersible inhibitor of intracellular and extracellular                   β_ lactamases, demonstrating concentration dependent and competitive inhibition.

Rationale of Combination –

Although clauvanic acid (CA) does have some degree of bacterial activity, its principle role as a beta – lactamase inhibitor. Beta – latame antibiotics, such as the pencillins and cephalosporins, act by disrupting the development  of bacterial cell walls thus causing the disintegration of the bacteria. However, some bacteria acquire the genes to produce enzymes which inactivate this mode of action and drastically reduces the effectiveness of antibiotics.

CA has a similliar structure to beta lactame antibiotics (pencillin and cephalosporin’s) but reverse of beta – lactamase enzymes. Used in combination with the beta – lactame antibiotics, it has become one of the most prescribed antibiotic prolonging the effectiveness of antibiotics.

Amoxicillin is found to be ineffective against bacteria which produces ESBL (Extended spectrum Beta – lactamase) enzyme and resistance is seen in such types of bacteria. The combination (Risiclav) provides a solution for treatment of bacterial infections caused by beta-lactam resistant pathogens.

Composition – 

Risiclav Tab,

 Amoxicillin – 500mg.

 Clauvanic acid -125mg.

Indication –

1. Un complicated UTI. Caused by   

 –  E.Coli.

 –  Proteuns.

2. Otitis Media. Caused by

 –  H. Influenzae (β_ lactamase +ve & -ve strains)

 –  M.Catarrhalis.

3. Pharyngitis and Tonsillitis Caused by

 –  S. pyogenes

4. Acute Bronchitis & AECB Caused by

  –  Strep Pneumonae

 –  H. Influenzae (β lactamase +ve & -ve)

5. Uncomplicated gonorrhea (Cervical / Urethral) Caused by

 –  N. Gonorrhoeae (β, lactamase +Ve & -Ve)

Dosage – 

Adult – One tablet twice daily for  7 to 14 days
